CENTRAL REGISTRATION AT WILMINGTON CITY SCHOOLS
Wilmington City Schools Central Registration office registers all new students entering Kindergarten through the Twelfth Grade for the entire Wilmington City School District. The office is located at Wilmington Middle School, 275 Thorne Avenue, in room 139, which is the first door in the hallway to your right as you enter the building.
The office is open for registration Monday through Friday from 8:00 a.m. to 2:00 p.m. For your convenience, and to minimize waiting time, it is advisable to call (937) 283-7440 for an appointment.
REGISTRATION REQUIREMENTS
The following documentation is required to register your child at Wilmington City Schools:
- Birth certificate or passport
- Immunization form (or records)
- Social Security card
- Custody papers (if applicable)
- Proof of Residency (mortgage/rent receipt, lease agreement, utility bill, or other document - drv. lic. not accepted)
- Grade Card or Transcript from previous school (required for high school)
- Name, address, phone and fax number of previous school
- Copy of I.E.P. (if applicable)
- Parent/Guardian drivers license and social security
