| Enter the clinic through the entrance at the rear of the building. |
| Morning clinics are every Tuesday, Wednesday and Thursday, 8:00-9:00 a.m. |
| Afternoon clinics are every Wednesday, 1:00-2:45 p.m. |
| Thursday evening clinics are on the second and fourth Thursdays of each month, 4:30-6:30 p.m. |

Tell Us About Nurse Connie

Nurse Connie has two degrees from Southern State and one from Wright State. She received her LPN and Associate degree in Social Sciences from Southern State and her BSN from Wright State.
She has been working for the Health Department in Wilmington for the past 15 years.
Nurse Connie didn't always want to be a nurse. In the beginning, she wanted to go to law school or study accounting. Luckily for us, she changed her mind!
If she weren't a nurse, she thinks she'd be a CPA or Tax Attorney.
Her favorite quote is from the Book of Proverbs in the Bible. "Trust in the Lord with all thy heart; lean not to thy own understanding. In all thy ways acknowledge him and he shall direct thy path."

Kindergarten Immunizations
|

| SHOTS ARE NO FUN! But, the Ohio Revised Code has established immunization requirements regarding entry into kindergarten. The State law requires the following immunizations: |
| 1. DTaP or DTP - Four or five doses. If all four doses were given before their fourth birthday, they must received a fifth dose. |
| 2. Polio - Three or four doses. If all three doses were given before their fourth birthday, they must receive a fourth dose. |
| 3. MMR (Measles, Mumps, Rubells) - Two doses. The first dose must have been administered after the child's first birthday, and the second dose, at least 28 days later. |
| 4. Hepatitis B - Three doses. The second dose must be administered at least 28 days after the first dose. The third dose must be administered at least two months after the second dose and at least four months after the first dose and at least six months of age. |
| 5. Chickenpox (Varicella) - One dose. Required or parent or physician statement that child has had the disease (include date). |
